The scripts for creating the missing indexs on the PIPELINES table are not placed in ORACLE_HOME/config/db/oracle/PolicyRepository/ but in ORACLE_HOME/owsm/config/db/oracle/PolicyRepository/, at least for the SOA Suite installation.
